Invisalign treatment with extraction to correct anterior open bite
Patient presents with a Class II division I malocclusion with excessive overjet, anterior open bite, mild upper incisor crowding and moderate lower anterior crowding.
Two upper first bicuspids were removed to allow the upper anterior teeth be retracted to close the anterior open bite.
Interproximal reduction was performed on all lower anterior teeth to correct the dental crowding.
Invisalign aligner was used to correct the lower dental crowding and to close the extraction space. The patient had braces for three months on the upper second bicuspids and the first molars to help improve the root angulations.
Important note: moving teeth over large distance, such as in this case with extraction, is less predictable with Invisalign aligners. Large composite buttons were placed on the teeth adjacent to extraction site to prevent excessive tipping of these teeth. Case selection is vital to ensure ideal treatment outcome. The patient has open bite, and extraction of teeth usually deepen the anterior overbite. The patient also has relatively smaller teeth and slightly shorter roots. Hence, it was easier to move those teeth.
